Diabetes patients have too much blood sugar, or glucose, in their blood. Since too much glucose can lead to serious health problems, controlling diabetes is vital to patient health. For this reason, ongoing monitoring of diabetes control is an essential part of our approach to this chronic disease.
We offer our patients comprehensive management of their diabetes. Our program includes:
- exercise
- medications
- lifestyle modification
- weight loss management, as needed
- on-site labs to manage and assess your diabetes
Our diabetic patients track and self-report their progress with diet and exercise. At their office visits, we discuss their progress as indicated by their logs and lab results. In addition, patients who enter one of our weight management programs may participate in online diet and calorie tracking. Then, our staff log in to the tracking tool to monitor progress.
At Carolinas Coastal Health, we closely track our diabetic patients' health status. We report 12 criteria for diabetes control each month to a national database that compares our care versus care across the area, state and nation to determine how we rank. Our practice ranks above average in virtually every category.
